Our community Wellness Clinic is staffed by licensed nurses and offers routine health services seven days a week without an appointment, including:

PVE @ Home for Residents
PVE at Home is a fee-for-service program that helps you live in your own home on our campus safely and independently. For this program, nonmedical caregivers are certified nursing assistants (CNAs) and employees of Paradise Valley Estates. They can help residents who need extra support with activities of daily living. Whether that’s temporary or ongoing care, our care coordinator will review your specific needs and develop a customized plan of services to help you.

As part of a Life Plan Community, Paradise Valley Estates’ Willow Creek residents live with the added security of daily support plus care for complex or chronic health needs. It’s ideal for those who would benefit from a higher level of Enhanced Assisted Living support than traditional assisted living provides but aren’t ready for skilled nursing care.
Willow Creek’s team of licensed and certified caregivers assists residents with needs such as incontinence, mobility, forgetfulness, medication management, or provides cues for activities of daily living such as bathing, dressing or eating. Regular social, physical and educational opportunities support friendships and interests of residents while making each day more enjoyable.

Our rehab health center, part of a picturesque, Northern California Life Plan Community, offers chronic care for complex health needs.
